Support Begins Before a Resident Moves In
At The Forum at Lincoln Heights, we work with each resident, listening to his or her unique needs and requests, and tailor a care plan that allows our residents to enjoy their retirement. We go farther than conventional retirement communities to create a perfect fit of support services for each resident, beginning with a customized assessment program, which measures a resident’s Assisted Living needs before moving in. It covers their need for support in a wide range of areas:

▪ Socialization and activities
▪ Continence management.
Each resident is assigned a specific level of care, based on the amount of assistance provided. We make sure the right services are in place for a successful transition, then we follow up within 30 days to make sure we are meeting the resident’s needs.
Ongoing Evaluation and Close, Personal Care
On a regular basis, we meet with the residents and their families to assess the resident’s need for assistance and support. Every resident also gets a quarterly wellness visit to monitor individual health benchmarks and medication needs. And because we also have expertise in skilled nursing health care, we bring that knowledge to our assisted living residents to make sure we’re providing them with the level of help they need, no matter how those needs may change.
