At The Forum at Lincoln Heights, we work with each resident, listening to his or her unique needs and requests, and tailor a care plan that allows our residents to enjoy their retirement. We go farther than conventional retirement communities to create a perfect fit of support services for each resident, beginning with a customized assessment program, which measures a resident’s Assisted Living needs before moving in. It covers their need for support in a wide range of areas:▪ Dining

▪ Managing medications
▪ Getting around
▪ Socialization and activities
▪ Continence management.Each resident is assigned a specific level of care, based on the amount of assistance provided. We make sure the right services are in place for a successful transition, then we follow up within 30 days to make sure we are meeting the resident’s needs.
Ongoing Evaluation and Close, Personal Care
On a regular basis, we meet with the residents and their families to assess the resident’s need for assistance and support. Every resident also gets a quarterly wellness visit to monitor individual health benchmarks and medication needs.
